Patient Policy

1. New patients are expected to arrive 15 min. prior to your appointment time in order to complete the needed paperwork.

2. Patient is to have hard copies of  medical reports prior to seeing Dr. Lee for consultation. 

3. Patient is responsible in providing her insurance information before seeing Dr. Lee. 

4. Copayment is to be collected before seeing Dr. Lee.

5. Patient is to treat provider and all staff in a cordial manner and with mutual respect.

6. Patient should respect each other in the waiting area and to keep talking volume down.
